Where the societies diverge

Grades. A randomized trial or meta-analysis of randomized trials. B cohort above 400 patients, or a society guideline. C smaller or confounded cohort, or physiologic reasoning without outcome data. INFERENCE is not a grade — it marks a conclusion drawn by reasoning over the cited material where no study addresses the question directly, and those blocks say so.

Fields appear only when they matter. An input is shown when changing it would change the recommendation given everything else currently selected. Tumor size below 4 cm, for instance, never alters the answer, so the sub-4 cm distinctions are not offered. Hidden fields can be revealed at any time.

Scope. This states what the published evidence supports for a population. It does not replace judgement applied to a patient and carries no authority over a written directive. Risk tiers are implemented from the 2025 ATA four-category structure; verify the low-intermediate / intermediate-high boundary against the criteria tables before adopting locally.
